Responsibilities:
Assist with Activities of Daily Living
Medication Reminders
Grooming, Bathing, Toileting
Keeping Patient Safe
Light Housekeeping
Prepare Meals/Asst with Feeding
Report any changes in client's condition to the office
Completes appropriate visit records in a timely manner as per agency policy.
Reports changes in patient condition to the office RN.
Qualifications:
Must be a Certified Nursing Assistant.
Must meet Medicare conditions of participation for Home Health aide training program and competency.
Must have complete a current health evaluation including: a 2 step PPD (dated within last 6 mos) or Quantiferon test (dated within last year), Drug Screen (dated within last 3 mos) and Rubella/Rubeola Titres (must include lab reports and results) 3. Work & Character References.
Proof of eligibility to work in the US (2 forms of ID)
